NHIF in Tanzania — what employers and employees need to know

The National Health Insurance Fund (NHIF) provides medical cover for civil servants, private-sector employees and the self-employed across Tanzania. The standard contribution is 3% from the employee + 3% from the employer of gross monthly salary, with payment due by the 9th day of the following month.

What does NHIF cover?

  • Outpatient and inpatient services at accredited hospitals nationwide.
  • Surgery (minor and major), maternity care, optical, dental.
  • Selected chronic-illness care including diabetes, hypertension and cancer.
  • Dependents — spouse plus up to four children under 21.

Who must contribute?

NHIF is mandatory for civil servants and parastatals. For private-sector employers it is mandatory under the Employment & Labour Relations Act for any business that opts into the scheme — and most do, because employer-paid medical cover is a strong retention lever.

  • Register the company with NHIF (one-time).
  • Enrol every employee — capture biometric details.
  • Compute 3% + 3% per employee per month.
  • Remit by the 9th of the following month via the NHIF portal or any TRA-listed bank.
